Safety Needles and Needlestick Protection: A Guide for Practices

Safety needles are disposable needles with an integrated protective mechanism that covers, encloses or retracts the needle tip after use. They are one of the technical measures against needlestick injuries. When selecting them, the main factors are the type of activation, compatibility with syringes, the range of sizes and easy handling for the whole team.
Why safety needles? The legal framework
Needlestick and cut injuries are a well-known risk in healthcare, because pathogens can be transmitted via used needles. In Germany, TRBA 250 (Technical Rule for Biological Agents in healthcare and welfare services) governs the protection of employees. It requires that sharp and pointed instruments with a safety mechanism are used for activities involving a risk of infection, as far as this is technically possible. At European level, Directive 2010/32/EU on the prevention of sharp injuries pursues the same goal.
Which activities are affected and which products are used is determined by each facility in its risk assessment, usually together with the occupational safety specialist and the company doctor. This article provides an overview for purchasing and does not replace advice in individual cases.
How do safety needles work?
All safety needles follow the same principle: after use, the needle tip is permanently shielded. Manufacturers use different mechanisms to achieve this.

A further distinction is made between active systems, where the protection is triggered by a hand movement, and passive systems, which secure the needle without an additional step. For acceptance in everyday work, it is important that the mechanism can be activated with one hand wherever possible and that locking is clearly noticeable.
Disposable needles, safety needles and safety syringes
Disposable needles, often also called single-use needles, are sterile needles for single use with a Luer hub, available for example under Single-use needles. Safety needles are disposable needles with an additional protective mechanism. There are also safety syringes with a permanently attached needle, butterfly needles and blood collection systems with a protective device, as well as pen needles with safety protection. In German-speaking markets these products are called "Sicherheitskanülen".
Sizes follow the same system as standard needles: outer diameter in gauge (G), length in millimetres and a colour code on the hub according to ISO 6009. Checklist: selecting safety needles
- Activation: possible with one hand, locking audible or tactile.
- Visibility: needle tip and bevel remain clearly visible with the shield folded back or pushed back.
- Size range: all required gauge sizes and lengths available in the same system.
- Compatibility: connector matching the syringes in use, see Luer Lock or Luer slip.
- Product documentation: CE marking, instructions for use in German, sterility and expiry date.
- Consistency: as few different systems in the facility as possible, so that instruction and handling remain simple.
- Test before switching: request samples where possible, trial them in the team and document feedback.
- Packaging unit: compare the packaging unit and individual packaging per supplier, as each supplier sets these itself.
Safe disposal: sharps containers are part of it
Safety needles are also disposed of after use in puncture-resistant, securely closable sharps containers. The containers should be available in a suitable size and sufficient number wherever needles are used. The fill line is marked on the container; full containers are closed and disposed of in accordance with local regulations.
